Lab Start-Up/Close-Out/Relocation

Laboratory Start-Ups

It is necessary for new principal investigators to communicate with the Occupational and Environmental Safety Office as soon as possible (prior to arrival if possible) to ensure regulatory compliance. Once notification is received, an OESO representative will contact the principal investigator to schedule an on-site visit to discuss all applicable safety policies and procedures.

Laboratory Close-Outs & Relocations

This procedure provides structured guidance for assuring the proper transfer and/or disposal of hazardous materials during a laboratory closeout. These closeout procedures apply both to those researchers that are permanently closing out their lab and also to those that are closing out their current lab space to relocate to a different location on the Duke campus.

Lab Equipment Hazard Assessment

All laboratory equipment must be free of hazardous materials (i.e. biological, chemical, radiological) prior to maintenance or transport to Duke Surplus, or other destinations.  Appropriate removal of hazardous materials will protect service personnel. Furthermore, it will help prevent Surplus from donating any hazardous laboratory equipment.

Hazard Assessment Form ( MS Word ; .PDF ); The equipment owner must sign and attach this form to any applicable lab equipment.
