STEP 4: If you are still experiencing pain or discomfort, seek appropriate medical treatment for an assessment of the symptoms as well as determination of the need for an ergonomic evaluation.

If you believe your pain is work-related, you are required to discuss this with your manager/supervisor and complete an injury/illness report within 24 hours.  Your manager/supervisor must also complete their portion of the form. For more information contact Workers’ Compensation or Employee Occupational Health(EOHW).
