Radiation Safety

Instructions for Ordering Radioactive Materials

ALL orders for radioactive material are placed through the Radiation Safety Division, under the oversight of Terry Mangum (668-3184) and Brian Gibbs (668-3182).When you place an order, please supply us with your order information below in one of these ways:

  1. Complete online and then simply click the 'Submit' button. Your order will be sent directly to us immediately.
  2. Download the form. Fill it out, save it on your computer and email it to us as an attachment to: radsafe@mc.duke.edu
  3. Download the form. Fill it out, and print it. Then fax it to us at: 668-2783.
  4. Hand deliver a hard copy to our office at 2214 Elder Street.

PLEASE NOTE: We will NOT transcribe data. If we cannot read the form, you will be called and asked to resubmit it. This may delay your order. J

A few pointers on filling out the form:

  1. "Vendor Name and Address:" If you are filling this form out on the Web, you will find a drop-down menu for this section. If your vendor is not listed, please email Terry Mangum and request your vendor be added to the list. You will need to provide him with the name, address, and phone number of the vendor you would like added.
  2. "Delivery Date:" In the top, right hand corner we ask for a delivery date. Yes, most of the time, 'normal' is synonymous with 'next day'. So, just to be safe, if you want next day, check next day. And, remember to also include any special delivery instructions in this box.
  3. Section Two: Be certain to fill in all the contact information in the second section of the form. If this section is incomplete, your order may be delayed. This information is crucial to insure proper ordering and delivery.
  4. "Purchase Order #:" Be sure to use a TEN-digit PO# received from Procurement or HHMI.
  5. Quote #:" Optional. Use this only if you have a special quote number only given from the vendor for your particular order. Do not put the general Duke quote number here.
  6. "Comments:" Be sure to supply any other information that we may need to know about your order.
  7. Isotope Order Information: "Catalogue #", "# of Vials", "Quantity/Unit", and "Measure", are all required for this line item to be processed. "Description" is not required. "Catalogue #" is the Vendors Catalogue Number. "Unit" is the number of vials that are being ordered. "Quantity/Unit" and "Measure" is the Isotopes Activity Per Unit Ordered.  Once the Isotope Order Information has been entered click the "Add Item to Request button. You may repeat Step G as many times as needed.

    Example: You want to order 20 Milli Curie of Carbon 14 .
    Catalogue #     Isotope Vials Quantity/Unit Measure Description
    123                      Carbon 14            1 20 mCi 
       
    456                     Carbon 14            2 10 mCi 
       
    Both these orders are the same as far as Quantity just packaged differently
  8. Click the Submit button to process the request.

CUT-OFF times to insure next day delivery. 
Please have your order to us by: 4:15 PM for next day deliveries.
