Welcome to the Standard Laboratory Procedures Template site. Use this form to generate a "template" of operating procedures for your laboratory. Each template is designed specifically for a particular Authorized User, based upon the properties of the radionuclides contained in that user's Duke Radioactive Materials Authorization.

To use this form:

(1) Select your Authorized User's name from the drop-down list.

(2) Fill in the field for "Radiation Survey Schedule". This field should contain information regarding the frequency at which surface "wipe" tests will be performed, and details about the method you use to document your survey results (sticky notes, log-book, forms in a three-ring binder, spreadsheet, database, etc.).

(3) When the form is complete, click the "Create Template" button. If you want to clear the form and start over, click the "Reset Form" button.

You may print the template directly from your Web browser*, or save it to your hard drive as an ".htm" or ".html" file. Most word processors will open these files. You can then edit the template as desired to produce a complete set of procedures specific to your laboratory. See the detailed instructions on saving these files to your hard drive.

*NOTE FOR PRINTING FROM BROWSER: For best results, go the "File" menu on your browser, then "Page Setup..." and supress printing of the title, URL, and page numbers. MS Internet Explorer will place page breaks between the major sections. This gives you the opportunity to insert your own material into the appropriate sections. Netscape Navigator will not allow page breaks; however, you can save the page to disk and insert the breaks manually with your word processor.
