Personnel Dosimetry
Duke employees who work with radiation sources or radioactive materials may be required to wear a personnel dosimeter ("film badge"). The criteria for issuing a dosimeter are based upon regulatory compliance considerations and individual circumstances. Decisions about monitoring individuals or groups are made by the Radiation Safety Officer.

Requesting Personnel Dosimetry
To request personnel dosimetry, please fill out the online form located here. You will have to log in with your Duke NetID and password to access this form. If you have any questions about the form, please contact us via phone at (919) 684-2194 or email at radsafety@dm.duke.edu.
Lost Dosimeter
If you lose your dosimeter, contact us to obtain a replacement as soon as possible. Do not wear another person's dosimeter, and do not wear a dosimeter marked as a "Control". If a replacement dosimeter is not needed immediately, you can download a Certificate of Lost Badge* form, and return it to us via email.
Pregnancy Declaration and Fetal/Embryo Monitoring
It is recommended that pregnant radiation workers formally declare pregnancy and receive appropriate radiation safety consultation as early as possible. Duke Employee Occupational Health and Wellness (EOHW) collaborates with the Radiation Safety Division to ensure that declared pregnant workers have radiation exposures below regulatory limits during the period of gestation.
